This role covers multiple locations in Elgin, Geneva, St Charles and West Chicago. A $1,000 Sign-On Bonus is offered for full-time employment.

What you will be doingThe Manager of Community Health Services is responsible to coordinate community health services for individuals in I/DD Service Programs for assigned regions according to DHS, HCBS, and CARF requirements. The essential job responsibilities include collaborating with internal and external health care providers in the implementation of individualized strategies that mitigate identified health risks and maximize the client’s progress toward selected outcomes and community inclusion.
Key responsibilities include, but are not limited to:
- Hire, train, supervise, and evaluate the Community Health Services team; ensure timely scheduling, completion, and documentation of routine, required, and urgent client medical appointments per applicable standards.
- Support clients who require intensive care including toileting and bathing, special diets with personal and/or enteral feeding, and enhanced funding; participate in direct service as needed.
- Maintain on-call responsibilities to ensure quality services 24/7 via an on-call rotation; model the organization’s core competencies; create and maintain a positive, professional, and safe environment for clients and staff.
- Lead and participate in service planning meetings and coordinate internal and external services to mitigate risks and maximize client outcomes and community inclusion.
- Coordinate medication administration processes, ensure documentation accuracy, and maintain compliance with licensure and regulatory requirements.
- Oversee documentation in client case records, audits, and monthly reviews; manage staff time records, payroll records, and expenses; address deficiencies with appropriate corrective actions.
- Ensure compliance with agency policies, procedures, and training regulations; provide ongoing supervision and feedback to staff; participate in program planning and quality assurance activities.
- Collaborate with supervisors and stakeholders to secure additional health services as needed; participate in emergencies and critical needs planning with solutions-focused approaches.

What will you bring to the tableEducation
High School Diploma or GED required;
Associate’s or Bachelor’s Degree in a Health Sciences or Human Services field preferred.
License/certifications
CNA or Medical Assistant certificate desired; QIDP training desired.
Experience
One year of experience working with developmentally disabled adults required (preferably as a Direct Support Professional); two to three years of supervisory experience and/or increasing responsibilities.
Physical
Climb stairs daily; no medical restrictions preventing lifting or transfer; push, pull, reach, and standing for extended periods; assist clients with transfers and activities of daily living; implement behavior support strategies and respond to emergencies.
Equipment
Proficiency with computer software (Word, Outlook, SharePoint, Excel, Teams); use of general office equipment; access to adaptive and medical equipment as needed.
Additional Requirements
Minimum age 21; ability to lift up to 50 pounds; pass literacy and specialized training requirements; EMT-B status with continuing education; valid Illinois driver’s license and access to a personal vehicle; ability to respond to client medical concerns during non-business hours; attendance at after-hours programmatic functions as required.
