The Association for Individual Development (AID) is a non‑profit organization whose mission is to empower people with physical, developmental, intellectual, and mental health challenges; those who have suffered trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for a Case Manager Mental Health Professional (MHP/QMHP) who demonstrates this mission and wants to work for an organization that makes a difference in the community.

Schedule: Mon–Fri 8:00 am–4:30 pm.

What you will be doing
  • Provide supportive case management services for Dundee Township residents, including temporary support with case management or therapeutic intervention services.
  • Implement referral processes for continuation of services and assist with allocating services to other providers to meet resident needs.
  • Execute client IM+CAN treatment plans, documenting services daily and ensuring client rights and dignity while maintaining a clean and safe environment.
  • Maintain compliance with all applicable program and agency regulations.

Job Responsibilities
  • Adhere to scheduled hours and remain flexible to meet client and program needs, including cross‑training and coverage in other programs as assigned.
  • Provide independent program coverage when scheduled.
  • Review program referrals for community case management, verifying residency, tracking contacts, and collecting outcome data.
  • Update service tracker with referrals, time allocation, and collaborative outreach documentation.
  • Host open office hours for walk‑in services es Episcopal Church in Dundee.
  • Meet minimum service hour provision productivity standards (MRO) monthly and complete required documentation in Cx360.
  • Meet with assigned clients regularly, turning in activity logs daily.
  • Acquire and maintain required trainings and certifications.
  • Provide services that meet clients in the Supported Living Program, aligned with IM+CAN and the Mental Health Recovery Model.
  • Maintain professionalism and healthy boundaries with clients, coworkers, and outside agencies.
  • Attend monthly clinical supervisions as per DHS requirement and monthly staff meetings.
  • Follow manager and case manager direction regarding client interactions and interventions.
  • Ensure compliance with all rules, regulations, and agency policies.
  • Recognize emergency situations, take appropriate action, and complete required paperwork.
  • Promote client wellness through coaching, counseling, consultation, motivational enhancement, and referral services.
  • Communicate with service partners—physicians, nurse practitioners, counselors, pharmacists, employers, community supports—to facilitate client wellness objectives.
  • Utilize appropriate resources and educational materials for wellness coaching.
  • Maintain proper client records in Cx360 per agency procedures.

What you bring to the table
  • Bachelor’s degree in Human Services or a related field ().
  • Experience working with mentally ill adults or in case management preferred; bilingual in Spanish preferred.
  • Physical ability to navigate stairs, no lifting restrictions, and ability to provide services in clients’ homes.
  • Equipment: computer (Microsoft Windows), copy machine, telephone with voicemail, and basic household appliances.
  • Personal automobile, valid driver’s license, and minimum liability insurance as per AID’s Personnel Policy.
  • Ability to drive clients in personal or agency vehicles, with flexibility for overnight, evening, weekend, and holiday hours as required.
  • Certifications in First Aid, CPR, non‑violent crisis intervention, CEU’s, and other relevant training.
  • Flexibility to cover unscheduled shifts and work at multiple locations if needed.
