We are seeking a compassionate and skilled Registered Nurse (Home Health) to join our client’s growing team in Elgin, Illinois. This position involves providing quality nursing care in patients’ homes, supporting their independence, and enhancing their health outcomes. The ideal candidate will have strong clinical skills, effective communication, and a commitment to patient‑centered care.

• Deliver professional nursing care and conduct comprehensive patient assessments in home settings
• Develop individualized care plans in collaboration with physicians and healthcare teams
• Educate patients and families on treatment plans, medications, and post‑care instructions
• Monitor patient progress and adjust care as needed to ensure optimal outcomes
• Maintain accurate and timely documentation according to regulatory standards
• Coordinate effectively with the care team to ensure seamless patient care

• Full‑time, Monday through Friday
• Flexible schedules, no long shifts, minimal weekend shifts annually
• Comprehensive 6‑week RN onboarding program
• This is a Pay Per Visit position

• Local travel within the Elgin area required (travel reimbursement provided)

• Associate degree or higher in Nursing
• Active Registered Nurse (RN) license in Illinois
• Minimum of 1 year professional RN experience (home health experience preferred)
• Current BLS and CPR certification through the American Heart Association (assistance available if needed)
• Valid driver’s license, reliable vehicle, and current auto insurance

• Prior experience in home health or community nursing
• Strong assessment, communication, and organizational skills
