The compensation offered to the candidate selected for the position will depend on several factors, including the candidate’s educational background, skills and professional experience.

Job Overview

The Community Health Worker will primarily maintain a presence in the community with patients to promote, motivate and improve healthy living. The CHW will assist patients in utilizing resources and accessing services to overcome barriers in order to meet their health goals.

Essential Functions
  • Meet patients in a clinic, hospital, ER, and where patients are located and conduct needs assessment using the Social Determinants of Health Tool.
  • Schedule weekly follow‑up calls and home visits to patients.
  • Help patients with social issues such as homelessness, substance abuse, hunger, transportation, and many other social issues that are impacting the member’s life.
  • Assist patients with organizing their records, making follow‑up appointments with Primary Care Physicians or Specialists.
  • Help patients with the tasks of getting medical care, and work on health goals by helping to plan healthy meals or finding time to exercise.
  • Help patients access resources, including quality of care, childcare services, food, housing, transportation.
  • Maintain relationships with Community Based Organizations.
  • Able to travel and meet the members where they are; may include local churches, homeless shelters, and members’ residences.
