The above reflects the anticipated salaried range for this position if hired to work. The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

The highly skilled and accomplished Case Management Team supports a variety of patient review and discharge planning needs. This role will support Medical/Surgical/ICU areas, requiring collaboration with the patient and family, nursing, physicians, and social workers to progress the treatment plan, and monitor clinical activity to ensure the patient progresses through the continuum of care. MCG guidelines are utilized to determine the appropriate level of care as well as continue stay to ensure proper reimbursement from payors.

• Active New Jersey”¯RN License or approved RN license required
• ASN or BSN required
• AHA BLS prior to start date required
• Minimum three (3) years of medical-surgical or other clinical experience required
• Case Management experience preferred
• National Case Management certifications and/or pertinent clinical specialty certification preferred
