Director of Social Work

Job Summary:

At Elizabeth Park Rehabilitation and Nursing LLC, a leading provider of residential, long-term, rehabilitation, and subacute care, we are seeking an exceptional Director of Social Work to join our dynamic team. As a key member of our leadership team, the Director of Social Work will play a vital role in shaping the social work services we offer to our patients and residents.

With a focus on patient-centered care, the ideal candidate will bring a deep understanding of the social work profession and a passion for delivering compassionate care.

• Oversee the Social Work department, ensuring the delivery of high-quality social work services to patients and residents

• Develop and implement social work programs and services aligned with the organization's mission and goals

• Collaborate with interdisciplinary teams to ensure patient-centered care and address social and emotional needs

• Recruit, train, and supervise a team of social workers and other support staff

• Develop and manage budgets, ensuring efficient use of resources

• Maintain compliance with regulatory requirements and accreditation standards

• Stay current with industry trends, best practices, and research to continuously improve social work services

• Participate in quality improvement initiatives and contribute to the development of policies and procedures

• LCSW

• At least 2 years of experience in social work, preferably in a long-term care setting

• Strong leadership and management skills, with the ability to build and maintain effective teams

• Excellent communication, interpersonal, and problem-solving skills

• Ability to work effectively in a fast-paced environment and prioritize multiple tasks

• Commitment to providing patient-centered care and promoting compassion, respect, and dignity

• Current licensure/certification as a Social Worker LCSW

• Strong knowledge of social work theory and practice, with a focus on patient-centered care and case management.
