*** Performs all patient evaluations, writes Plan of Care in accordance with MD.
* Completes required documentation and submits within the allotted timeframe.
* Maintain standards of quality in accordance with accepted standards and hospital and departmental policies and procedures.
** Requirements:
*** Kentucky license as a physical therapist required. Requires a Bachelor, Master or Doctoral degree from a physical therapy program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE), or higher.
* BLS preferred at hire, required within 90 days after hire.
