$1,500 Sign‑On Bonus

During a visit up to one hour, providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. The Matrix Comprehensive Health Assessment (CHA) helps improve quality of care and close multiple care gaps in a single visit. The provider:

• Conducts adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case‑management referrals, follow‑up and clear documentation according to Matrix guidelines and protocols.
• Administers and orders point‑of‑care and lab‑based testing as needed.
• Works with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Collaborates with Primary Care Physician (PCP) on patient education and follow‑up as necessary.
• Provides services at the member’s place of residence, which can be in their home or a Skilled Nursing Facility.

• Flexibility – Visits are scheduled based on YOUR availability. Assessments are scheduled Sunday‑Saturday from 8 am‑7 pm. You can work during the day, evening, and/or weekend based on your schedule.
• Competitive Pay – Earn income based on the number of assessments you complete. Those completing a monthly average of 20–100 visits earn $1,800–$11,500 per month. Rate per assessment is $90–$115 depending on your state of residence.

• Education and Support – Comprehensive onboarding training, a preceptor, 1:1 feedback, and free access to continuing education.
• Additional Licensure Support – We support obtaining additional state licensure and credentialing in neighboring states.
• Full‑time Opportunities – PRN providers can transition to FT roles, gain a sign‑on bonus, and work at their home location or in a state or region.

• Master’s degree or commensurate experience and successful NP licensure.
• Current NP licensure in state of practice that includes prescription authority or ability to obtain prescriptive authority.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• Preferred 1 year of NP experience; new grads encouraged.
• Valid state driver’s license and proof of adequate automobile insurance coverage for the state of residence.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.
