The Targeted Case Manager’s primary duty is to provide case management services to families in the Family Recovery Court Program. TCM will assist individuals/families with obtaining the required resources to meet their needs, including housing, medical, social, educational, and other identified services.

• Provide case management services for families identified as needing targeted support.
• Conduct comprehensive assessments and reassessments to determine the need for housing, medical, social, and educational services.
• Develop an individualized case plan based on assessment findings, specifying goals and activities that address identified services.
• Administer Program Evaluation Assessments and observed urine drug screens on a pre-determined schedule. Refer clients to needed services and arrange transportation if necessary.
• Monitor and follow up with clients, family members, and service providers; update case plans as needed.
• Identify community resources and develop cooperative relationships with other social service providers.
• Represent agency programs, policies, and procedures accurately in the community and with other agencies.
• Maintain accurate case records and ensure documentation complies with KY Medicaid and accreditation standards. Attend Family Court and Family Recovery Court dockets weekly.
• Conduct Life Skills classes with participants in Phase 3 of the program.
• Participate in weekly case conferencing and provide updates on all assigned individuals/families.
• Participate in bi-monthly supervision with TCM billing supervisor and weekly supervision with direct supervisor.
• Attend monthly staff meetings, training courses, and annually required training to maintain TCM certification. Maintain certification requirements for TCM pursuant to Kentucky Board regulations.
• Assist with special projects and reporting requirements as assigned.
