JOB SUMMARY AND QUALIFICATIONS
:
The Targeted Case Manager’s primary duty is to provide case management services to families in the Family Recovery Court Program. TCM will assist the individual/family with obtaining access needed to support their needs.
The Case Manager must be able to assist the individual/family with obtaining needed access to housing, medical, social, educational and other identified services.
RESPONSIBILITIES:
Conduct comprehensive assessments, and reassessments to determine the need for housing, medical social, education and other identified services.
