SUMMARY:
Providesservicecoordinationtotargetedpopulationclients(Childor Adult).
QUALIFICATIONS:Bachelor’s degree in psychology, social work, or mental health; preference for work experience with the targeted population assigned within the human service setting. Minimum one (1) year of experience post degree working with persons in a human services setting. Must provide own transportation to provide home visits.
SKILL SETS:Adequate communication skills, process verbal and written instructions, dependable, interact effectivelywiththepublicandstaff,adaptability-abletodealwithfrequentchange,delaysorunexpectedevents, strong sense of commitment.
PHYSICAL AND ENVIORMENTAL DEMANDS:Visual acuity necessary for updating files and completing documents. Requiresfrequentstooping,kneelingandcrouchingto file records. Hearingandtalkingisnecessary toanswerphoneandtalkwithemployeesandotherindividuals. Workislightwithwalking,standingandsitting. Manual dexterity is required to perform work processing and other clerical duties. Possible exposure to blood borne pathogens, biohazardous waste, hazardous waste and communicable diseases.
JOB SPECIFIC FUNCTIONS:1. Conductshomevisits toassessservicecoordinationneedsfor clients.
3. Assistsclientsinaccessingallneededserviceswithinthe community
4. Performsadvocacyon behalf of clients
5. Providescase consultationstoservice providers
6. Provide pre-tenancy and tenancy supports to assist participants in identifying, selecting, and entering into a lease agreement resulting in the participant moving into an independent housing unit.
7. Performsotherdutiesas assigned.
