Our innovative team nursing model pairs an RN with an LVN and therapists, and CHHAs. We travel to patients' homes throughout the day.

• Provide optimum care to homebound patients and supervise medical necessity review, initial assessments, continued stay review, discharge planning, and other duties as required.
• Travel to patients' homes to provide care, complete and submit accurate electronic clinical notes, administer medications and treatments as prescribed by the physician, assist patients with activities of daily living, and teach patients and families self‑care techniques and medication and diet instructions while maintaining a safe environment.
• Perform additional job‑related tasks as requested.

• Graduate of an approved school of professional nursing, with a BSN preferred.
• Currently licensed to practice as an RN in the state of practice.
• Clinical knowledge of various disease processes.
• One year of acute nursing experience in medical, surgical, and/or critical care; home‑health experience preferred.
• Excellent observation, good nursing judgment, and good oral and written communication skills.
• Self‑directing, flexible, and cooperative in fulfilling role obligations.

Pay per visit ranges from $50 to $160, commensurate with experience and depending on type of visit. The compensation reflected on this posting is an estimate of yearly compensation.
