Home Health Licensed Vocational Nurse (LVN) San Joaquin County ***$5k Sign On Bonus***

Join our team as a full‑time Licensed Vocational Nurse (LVN) in San Joaquin County. Lorian Health, a home health and hospice agency, is seeking an energetic LVN to provide direct nursing care to clients across Stockton, Lodi, Tracy, Manteca, Lathrop, and French Camp.

Qualifications:

• LVN/LPN licensed in the state of practice and in good standing.
• Graduate of an approved school of practical/vocational nursing.
• One year of experience as a professional nurse in an acute setting.
• Current CPR certification (BLS required).
• Valid driver’s license and insured automobile meeting Lorian requirements.
• Ability to read and follow written instructions and document care in the electronic medical record.
• Excellent observation, nursing judgment, and communication skills.
• Self‑directing with minimal supervision.
• Ability to work with adults (ages 18+) demonstrating tact and patience.
• Flexible and cooperative in all role obligations.
• Participates in in‑service education programs and clinical meetings.
• Adheres to infection‑control policies and protocols.
• Maintains confidential patient records per HIPAA and applicable state laws.

Responsibilities:

• Provide direct nursing care to clients as prescribed by the physician, under RN supervision.
• Travel to patients’ homes throughout the day, documenting care via a smartphone provided by Lorian Health.
• Observe patients for changes in condition, discuss changes with patient/family, and report to the attending physician.
• Educate patients, families, and caregivers regarding care and self‑care practices.
• Maintain accurate and timely documentation in the electronic medical record.

Compensation & Benefits:

• Pay‑Per‑Visit model: $30/visit – $40/visit, commensurate with experience.
