The Home Care Nursing Manager provides strategic and operational leadership for a high‑performing home care department, ensuring the delivery of exceptional, patient‑centered nursing services. This role oversees clinical quality, staff development, and compliance with established workflows and policies while supporting nurses caring for frail and elderly populations across multiple care settings. This is an outstanding opportunity for an experienced Registered Nurse leader who is passionate about quality care, team development, and improving outcomes for older adults.
Duties- Lead and direct a high‑performing home care department, ensuring workflows, clinical standards, and policies are consistently followed.
- Guide, support, and manage home care staff, including hiring, onboarding, training, coaching, and ongoing performance development.
- Assess the quality and effectiveness of home health nursing assessments and team participation.
- Ensure home care nurses deliver, document, and follow through on nursing interventions as outlined in patient care plans.
- Oversee the quality of nursing care provided to patients residing in nursing homes and board‑and‑care facilities.
- Monitor clinical practices to ensure safe, effective, and compliant care delivery across all settings.
- Promote collaboration, accountability, and excellence within the nursing team.
- Other duties as assigned
- Active California Registered Nurse (RN) license.
- Minimum of two (2) years of nursing experience in a home health setting, preferably serving frail or elderly populations.
- Strong clinical knowledge required to care for medically complex, elderly patients and manage intricate clinical scenarios.
- Demonstrated leadership skills with the ability to guide, coach, and support clinical teams.
- Bachelor’s degree preferred; additional relevant experience may be considered in lieu of formal education.
- Prior leadership or supervisory experience in home health or community‑based nursing.
- Experience working within integrated, team‑based care models.
