Physician - Family Medicine with OB - Elk River, MN

Job Overview Fairview is seeking a Family Medicine with OB physician to join our primary care practice at M Health Fairview Clinics – Elk River located in Elk River, Minnesota. This clinic is a conveniently located community clinic on Main Street in downtown Elk River that offers comprehensive care for your entire family, through all stages of life.

Practice Details:15 provider primary care practice which includes 10 Physicians and 5 Advanced Practice Providers. Patient encounters average 15-18 patients per day.

Opening: 1.0 FTE - 34 patient contact hours per week for full time - flexible schedule. Call Frequency: 3 weeks per year.

As part of Fairview’s Accountable Care Organization (ACO), providers practice in a collaborative, team-based model focused on quality outcomes, care coordination, and population health management.

Qualifications:
