Paediatric Diabetes Specialist Nurse

Paediatric Diabetes Specialist Nurse – Calderdale and Huddersfield NHS Foundation Trust (CHFT), Rainbow Community Hub, Elland. 37.5 hours per week (up to 37.5 hours). Band 6; permanent contract.

We are seeking a Paediatric Diabetes Specialist Nurse to join our award‑winning, multidisciplinary team. The role involves advanced diabetes care using technologies such as closed‑loop insulin pumps and continuous glucose monitoring, with care delivered across Calderdale and Huddersfield, including home visits, school settings and clinic appointments.

• Provide evidence‑based specialist care to children and young people with Type 1 and Type 2 diabetes, including dose adjustments and telephone support.
• Manage an individual caseload, make daily clinical decisions and support families in establishing independent diabetes management.
• Attend multidisciplinary clinics and meetings (weekly), regional nursing network sessions and safeguarding meetings.
• Initiate and continue pump therapy, delivering structured education and information to patients and carers.
• Provide education to families, school staff and other professionals involved in patient care.
• Compile and analyse audit data to support service improvement.

• Registered Nurse (RN) – child nursing or equivalent.
• Current NMC registration.
• Professional or clinical qualification relevant to paediatric diabetes (e.g. Children’s Diabetes Modules) or evidence of working towards one.
• Degree level education or higher.
• Willingness to undertake Non‑Medical Prescribing and Insulin Pump Therapy courses.

• Experience caring for children and young people with diabetes.
• Evidence of continual professional development and up‑to‑date research and evidence‑based practice.
• Knowledge of NICE guidance and best‑practice tariff for diabetes care.
• Skill in assessment, planning, implementation and evaluation of care.
• Data protection and organisational skills.
• Experience working in a community setting managing a nursing caseload.
• Teaching/training other staff and awareness of clinical governance.
• Knowledge of relevant legislation, investigative training and root‑cause analysis.
• Counselling skills and willingness to learn new skills and implement change.

• Ability to work autonomously and as part of a team.
• Capacity to take charge and coordinate the team in the absence of the Clinical Manager.
