Responsibilities
Provide outpatient rehabilitation services in an in‑home setting for the older adult population, which may include patients’ private residences or independent living facilities.Evaluate and treat patients in the most functional environment, delivering individualized physical therapy programs to facilitate rehabilitation.Identify and make referrals to other disciplines to address comprehensive patient needs.Implement individualized therapy programs designed to restore, reinforce, and enhance performance.Instruct, educate, and train patients and caregivers in skills essential for promoting independence and productivity.Direct activities to promote and maintain health creating a safe environment and eliminating fire and safety hazards.Order and maintain supplies and equipment as necessary, cleaning and reporting equipment repairs.Screen, examine, evaluate patients (history, systems review, tests, and measures), and develop appropriate treatment goals and methods in collaboration with patients and caregivers.Implement the physical therapy treatment plan and complete all related documentation and records.Perform therapeutic interventions including therapeutic exercise, functional training, manual therapy, airway clearance techniques, integumentary repair, and electrotherapeutic modalities.Incorporate health and wellness programming and education on chronic disease management, fall assessment, and case management in the home and community.Design, fabricate, fit, and train patients in the use of adaptive devices, orthotics, and prosthetics.Instruct patients in mobility activities and evaluate progress, attitudes, and behaviors in rehabilitation.Perform routine reexaminations to modify or progress treatment plans.Conduct home and community mobility assessments and provide safe mobility recommendations.Attend and contribute to patient care, staffing conferences, and related meetings.Ensure adherence to department and patient treatment schedules and timely clinical and billing documentation.Perform other related duties as required.Qualifications
Must have a valid Physical Therapy license in the state(s) of practice or proof of license eligibility.Must hold a Master’s degree in Physical Therapy, or a Master’s in a related field plus a Bachelor’s in Physical Therapy.Alternatively, a Bachelor’s in Physical Therapy plus five years of progressively responsible PT experience.For degrees earned outside the U.S., the qualification must be evaluated as the U.S. equivalent to a Physical Therapy degree.Must have a valid driver’s license or reliable, independent transportation to and from patients’ residences.Posted Salary Range
USD $65.00 – USD $66.00 /Hr.
