Must have a valid Physical Therapy Assistant license in the state of Illinois

• Deliver patient care in accordance with the Physical Therapist’s plan of care and within the scope of practice as defined by state law and the State Board of Physical Therapy.
• Execute daily treatment sessions under the supervision and guidance of the Physical Therapist, ensuring high-quality, goal-oriented care while tailoring care to the patient.
• Provide physical therapy services in compliance with all applicable state regulations, professional standards, and agency policies.
• Utilize diagnostic equipment and monitoring systems for the weight loss program.
• Complete evaluations, progress notes, daily plan of care, and discharge documentation in an EMR system.
• Provide continuous communication with referring physicians/nurse practitioners, and the rest of the multidisciplinary team.
• Discuss goals and treatment plans with patients and record patient conditions and responses to treatments.
• Support daily clinic needs, including answering phones, on-boarding patient intakes, and maintaining clinic appearance.
