Cross County Medical Care is an established primary care and multi‑specialty medical practice with locations in Bellerose and Elmont, NY. We provide compassionate, high‑quality health care to our patients. We are seeking a full‑time Family Nurse Practitioner (FNP) with strong clinical skills and experience in primary care to join our practice. The ideal candidate will be comfortable working independently, managing a broad range of acute and chronic conditions, and collaborating with our medical and administrative teams to provide exceptional patient care.

This position will rotate throughout our locations, as needed.

The work schedule will be Mondays to Fridays and alternating Saturdays if needed.

• Conduct wellness visits, sick visits, urgent care, and follow‑up visits.
• Diagnose, develop treatment plans, and manage patient care effectively.
• Prescribe medications and monitor patient adherence and outcomes.
• Order and interpret diagnostic tests (EKG, PFT, ultrasound, x‑rays, etc.).
• Administer preventive care services.
• Refer patients to specialists when necessary.
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Participate in clinical quality improvement programs to enhance patient care.
• Work collaboratively with the Medical Director, clinical staff, and administrative team to ensure smooth practice operations.

• Must have an active NYS Nurse Practitioner (NP) license.
• Board Certification as a Family Nurse Practitioner.
• 3 to 5 years of direct patient care experience as a Nurse Practitioner necessary.
• Active New York State (NYS) Nurse Practitioner license required.
• Active DEA number needed.
• Active NPI number necessary.
• Malpractice insurance.

• Prior experience in private practice or primary care settings is a plus.
