Our structure is set up to provide Home Health Nurses multiple avenues for support that aren’t available at other agencies – a field preceptor, an IT specialist, a dedicated scheduler, a nurse preceptor during orientation (available every day to help with documentation questions and challenges), an in‑office team of QI specialists, telehealth resources, clinical manager and in‑office triage support. Position Overview

As a Home Health Speech Therapist
, you will join the Banner Home Health Clinical Team providing exceptional, expert care in your community. You will enjoy an autonomous practice focusing on functional rehabilitation within the patient’s actual home environment reflective of your patient’s specific needs. The Home Health environment allows for the time to create relationships with your patients and create your own schedule that is not possible in a hospital setting.

Be part of the Banner Home Health Team focusing on getting patients back to their daily life by providing physical and emotional needs during illness and through recovery.

This a Part‑Time position with a minimum weekly commitment of 3/8 hour shifts (24 hours per week). Flexible schedule option of 10 hour shifts available. No weekends or Holidays. This position will provide 1:1 home care in the East Valley (Maricopa, Casa Grande, Eloy and Arizona City).

This position requires a current license.

Position

Summary

This position provides screening, evaluation, and rehabilitation services for patients with speech and language disorders related to speech, language, cognitive‑communication, voice, swallowing, and fluency. This position plans and carries out specialized treatment programs, documents treatment, and maintains data regarding patient care. This position demonstrates the clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served.

  • Evaluates patients’ functional abilities and limitations to determine appropriate intervention. Develops a plan of care and treatment goals, as ordered by physician or other referral source.
  • Implements individualized treatment plans that consider patients’ physical, social, spiritual, cultural, educational, and age‑specific needs. Ensures that treatment plans are supported by standard practices and/or current literature. Re‑evaluates treatment plans, as needed, to ensure maximum patient benefit.
  • Educates patients, family, and other care providers to promote patient participation and attainment of therapeutic program goals. Provides individualized home programs with clear instructions.
