The Level 1 Practice Nurse position is suitable for a newly qualified nurse or nurse new to primary care. The role requires resilience and the ability to be flexible and adaptable. You will be supported to develop confidence to work alone without direct supervision, undertaking and reporting autonomous decisions made in practice. Newly registered nurses will be supported through a period of preceptorship.

Mereside Medical is a flourishing group of three East Cambridgeshire practices in Ely, Haddenham and Soham, together serving c.40,000 patients. The nursing team is responsible for the delivery of practice nursing services and care to the practice population. Supported by Nurse Manager and team members within the practice, they deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long‑term conditions, health promotion and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

We are embedded into the local community, constantly working to bridge the gap between primary and secondary care creating a seamless service for patients with long‑term chronic conditions, including those living in care homes.

Job responsibilities

Your Responsibilities

Clinical Practice and Quality

Demonstrate developing competence and clinical skills in the assessment of patients in the general practice setting, ensuring care is planned in partnership with patients and all information is clearly and objectively recorded according to practice policy.

To do this you will be required to:

  • Practice in compliance with your respective code of professional conduct and scope of practice, being responsible and accountable for your decisions, actions, and omissions at this level of practice
  • Work towards the Foundation Programme, Fundamentals of Practice Nursing order demonstrate equivalent experience spanning:

o Treatment Room

o Child Immunisations

o Cervical Cytology

o Complex Dressings

  • Assess patients, taking into account their physical, mental and social states, alongside the impact of their environment and support available to them, to produce a person‑centred plan of care
  • Demonstrate knowledge of a broad range of long‑term conditions and local care pathways, including for example, diabetes, respiratory disease, coronary heart disease
  • Demonstrate effective communication skills to optimise patient satisfaction while managing their expectations and supporting them to make decisions, plan care, or make positive change, including:

o When dealing with sensitive or confidential matters that may also have legal or ethical considerations, for example, safeguarding, substance abuse, sexual orientation and gender identity, racial and ethnic diversity, mental illness, disabilities, etc.

o To support patients to adopt health promotion strategies, and apply principles of self‑care, including referring or sign‑posting to other services that may support their wider well‑being, e.g. District Nurses, Social…
