Job overview

The Band 6 CYP Continuing Care Assessor is responsible for carrying out high‑quality assessments for children and young people with complex, ongoing health needs, ensuring eligibility decisions are fair, transparent, and aligned with the national Children & Young People’s Continuing Care Framework. The postholder works directly with families and multidisciplinary partners to build a clear, holistic picture of each child’s needs and contribute to safe, person‑centred care planning.

This is an operational clinical role that requires strong assessment skills, sound professional judgement, and the ability to manage sensitive situations with confidence and empathy. The Assessor gathers and interprets evidence, contributes to Decision Support Tools or equivalent frameworks, and produces clear recommendations that support robust and defensible decision‑making. The CYPCC Assessor works closely with health, social care, education, SEND, and safeguarding colleagues, helping ensure assessments are well coordinated and that families receive accurate, timely communication throughout the process.

They manage a defined caseload and escalated complex or high‑risk situations where senior clinical input is required.

We are currently looking for 6 WTE CYPCC Nurse Assessors to work across the Central East area which includes Cambridge and Peterborough, Bedfordshire and Hertfordshire.

Main duties of the job
  • Undertake holistic assessments of children and young people with complex health needs using the national CYP Continuing Care Framework.
  • Gather, review, and interpret health, social care, education, and family evidence to support accurate eligibility recommendations.
  • Contribute to the development of clear, person‑centred support plans that reflect assessed needs and follow best‑practice standards.
  • Manage a defined caseload, ensuring assessments, reviews, and documentation are completed within expected timescales.
  • Work closely with families, providing clear, compassionate communication and ensuring they understand the assessment process and outcomes.
  • Liaise with MDT partners, including local authority teams, SEND services, education, safeguarding, and health providers, to coordinate evidence and support planning.
  • Produce high‑quality, defensible assessment documentation that meets governance, legal, and audit requirements.
  • Participate in MDT discussions, case reviews, and decision‑making forums, contributing clinical insight and supporting consistent practice.
  • Identify safeguarding concerns or clinical risks and escalated them promptly in line with policy and local procedures.
  • Escalate complex, ambiguous, or high‑risk cases to senior colleagues for oversight or additional review.
