Pi-groep in Emmeloord zoekt een zorgcoach die regie kan nemen en samenwerkt in complexe hersteltrajecten binnen beschermd wonen.
Je coördineert zorgtrajecten, voert intakes uit, stelt begeleidingsplannen op en bewaakt de voortgang in nauwe samenwerking met collega's en externe partijen. Een HBO-diploma in zorg is vereist, met 2–3 jaar GGZ-ervaring; 24 uur per week beschikbaarheid; kennis van WMO/WLZ en autisme is een pré; rijbewijs gewenst.
