Overview
Optum is seeking a Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Enfield and Tolland, CT on a per diem basis (~24 hours per month).

As a team member of our Senior Community Care (SCC) product, you will provide care to patients at home in a nursing home or assisted living setting. This work adds a layer of support to improve access to care.

We are expanding to support Optum members and advance your advanced practice nursing career. As a CCM Nurse Practitioner/Physician Assistant per diem you will deliver medical care on a periodic basis.
Primary Responsibilities- Deliver cost-effective, quality care to assigned members
- Manage medical and behavioral, chronic and acute conditions in collaboration with a physician or specialty provider
- Perform comprehensive assessments and document findings compliant with documentation requirements and CMS regulations
- Ensure diagnoses are ICD-10 coded and documented to support the visit
- Address quality elements in documentation and care planning
- Conduct initial and annual medication reviews and post-hospitalization medication reconciliation
- Facilitate agreement and implementation of the member’s plan of care with facility staff, families, and primary/specialty care physicians
- Evaluate and revise the plan of care as needed
- Utilize CCM guidelines and protocols
- Attend and complete mandatory educational and Learn Source training requirements
- Travel between care sites is mandatory
- Understand payer/plan benefits and CCM policies; articulate them to providers, members and decision-makers
- Assess medical necessity/effectiveness of ancillary services and communicate processes to providers and team
- Coordinate care as members transition through different levels of care
- Monitor member and family needs and adjust the plan of care as conditions change
- Review orders/interventions for appropriateness and outcomes to align with member needs
- Evaluate plan of care for cost effectiveness while meeting patient, family and provider needs
- Maintain regular and effective communication with internal and external parties
- Promote the CCM program in facilities and support membership growth
- Demonstrate initiative and independent practice
- Enter available hours into the web-based application at least one month in advance
- Participate in CCM quality initiatives
- Be available to check Optum email for trainings and scheduling
- Active national NP or PA certification or ability to obtain by start date
- For NPs:
Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care; or for PAs – NCCPA certification - Active and unrestricted APC license in the State of Connecticut
- Current DEA licensure/prescriptive authority or ability to obtain post-hire (where allowed)
- Driver’s license and reliable transportation to travel to sites
- Availability to work 24 hours per month, with up to 16 hours during off-hours (after 5 pm, weekends, holidays); not to exceed 960 hours per calendar year
- 1+ year of hands-on post-grad experience in Long Term Care
- Ability to gain a collaborative practice agreement if required in your state
- Ability to lift 30 pounds and navigate stairs and various dwelling configurations
- Understanding of geriatrics, chronic illness, advanced illness and end-of-life discussions
- Proficient documentation skills in EMR and written records
- Ability to develop and maintain positive customer relationships
- Adaptability to change
Pay is based on factors including location, education, experience, and certifications. The hourly pay for this role ranges from $43.22 to $77.21 per hour (based on full-time equivalence). Benefits may include a comprehensive package, incentive programs, equity stock purchase, and 401(k) contribution, subject to eligibility.
