Position: Physical Therapist – Englewood, CO

Here at PT of the Rockies, we are seeking a dedicated and skilled Physical Therapist to join our team. The ideal candidate will be responsible for evaluating, planning, and implementing treatment programs for patients with various physical ailments. This role focuses on providing high‑quality outpatient care, ensuring that each patient receives personalized attention and effective rehabilitation strategies. The Physical Therapist will work closely with patients to help them achieve their recovery goals and improve their overall quality of life.

Duties
  • Conduct thorough assessments of patients’ physical abilities and limitations.
  • Develop individualized care plans based on assessment findings and patient goals.
  • Implement therapeutic interventions including exercise therapy, manual therapy, and trigger point therapy.
  • Provide education to patients and their families regarding treatment plans, exercises, and home health strategies.
  • Monitor patient progress and modify treatment plans as necessary to ensure optimal outcomes.
  • Collaborate with other healthcare professionals to coordinate comprehensive care for patients.
  • Participate in discharge planning to facilitate a smooth transition from therapy to home or other care settings.
  • Maintain accurate documentation of patient evaluations, progress notes, and treatment outcomes.
Skills
  • Strong knowledge of anatomy and physiology relevant to physical therapy practices.
  • Proficiency in outpatient physical therapy techniques and methodologies.
  • Experience in developing effective care plans tailored to individual patient needs.
  • Familiarity with discharge planning processes for various patient demographics, including geriatrics.
  • Ability to provide compassionate patient care while maintaining professionalism in a clinical setting.
  • Experience with home health services is a plus.
  • Excellent communication skills for effective interaction with patients, families, and multidisciplinary teams.
  • Strong problem‑solving skills to address challenges in patient rehabilitation effectively.
