Description

We are seeking an experienced and dependable LPN / Resident Care Coordinator (RCC) to join our clinical leadership team. This role is hands‑on and requires someone who is comfortable balancing resident care coordination, medication oversight, and floor support when needed.

This is a working nurse role, not a desk‑only position.

Schedule & Pay
  • Sunday–Thursday
  • Hourly pay range: $33–$36 per hour (based on experience)
Location

Assisted Living / Memory Care Community

Key Responsibilities
  • Provide direct nursing care to residents in Assisted Living and/or Memory Care
  • Administer medications and treatments in accordance with physician orders and state regulations
  • Serve as RCC by coordinating resident care plans, assessments, and clinical documentation
  • Supervise and support Caregivers/QMAPs during assigned shifts
  • Assist on the floor as needed with resident care and staffing support
  • Communicate effectively with families, providers, and internal leadership
  • Ensure compliance with Colorado Department of Public Health and Environment (CDPHE) regulations
  • Participate in audits, surveys, and quality assurance processes
  • Respond to clinical concerns, incidents, and emergencies appropriately
Qualifications
  • Current Colorado LPN license in good standing (required)
  • Experience in Assisted Living, Memory Care, or Long‑Term Care preferred
  • Strong medication management and clinical judgment skills
  • Ability and willingness to assist with hands‑on resident care
  • Experience supervising or leading care staff strongly preferred
  • Strong documentation and communication skills
  • Ability to remain calm, organized, and decisive in a fast‑paced environment
