Description:
Travel RN Case Manager position available.
Responsible for managing case load on the unit.
Team consists of Case Managers and Social Workers.
Patient ratio for Social Workers is 1:40.Patient ratio for RNs is 1:20.Utilizes electronic medical record system for documentation.
Key responsibilities include discharge planning and assessments.
Experience in discharge planning is required.
Knowledge of palliative care and Goals of Care conversations is preferred.
Candidates should possess strong communication skills.
Candidates should be adaptable to different units.
The best personality fit is someone who is professional, dedicated, and friendly.
Candidates with previous travel RN experience will be preferred.
