Lead MDS Nurse; LPN; Care Plan & IDT Leader

Pavilion Management Company is seeking an MDS Nurse (LPN) for a full-time, on-site role in Ohio. The nurse will chair the interdisciplinary care plan team and develop plans of care for residents, establishing initial plans within five days of admission and updating them biweekly.

Responsibilities include coordinating MDS assessments, leading weekly Medicare meetings, monitoring IDT compliance, and ensuring timely billing. Licensure, MDS experience, leadership, and CPR certification are required.
