Gentiva's Southern Care division is seeking a Hospice RN / RN Case Manager to lead in-home, hospice care. You will perform comprehensive assessments, develop individualized plans of care, and deliver skilled nursing services to patients in their homes, assisted living, or wherever they call home.
Collaborating with an interdisciplinary team, you’ll educate families, coordinate referrals, document promptly, and support bereavement needs.
