Position: Registered Nurse Assessment Coordinator  | 40 hours week
Registered Nurse Admissions & Assessment Coordinator (RNAC)

Department: Healthcare
Reports to: Director of Nursing
Status: Non-Exempt
Pay Range: $46.00 $ 50.00 per hour

Fairmount Homes, a faith-based Continuing Care Retirement Community founded on Christs love and Mennonite values, is seeking an experienced Registered Nurse Admissions & Assessment Coordinator to join our Healthcare team.

Position Summary

This role is responsible for coordinating timely, accurate, and compliant MDS assessments, Care Area Assessments (CAAs), and interdisciplinary care plans. The RN Admissions & Assessment Coordinator oversees healthcare admissions, supports Medicare eligibility and documentation, facilitates IDCP meetings, and serves as a clinical and regulatory resource for the interdisciplinary team.

Key Responsibilities
  • Coordinate and manage MDS, CAAs, care plans, and RAI submissions per state and federal regulations

  • Maintain the MDS calendar and ensure timely electronic transmissions

  • Collaborate with the interdisciplinary team on Medicare eligibility, skilled needs, and care planning

  • Review and respond to admission referrals and oversee the admissions process

  • Ensure accurate Medicare documentation, HIPPS codes, ICD-10 coding, and physician certifications

  • Monitor Quality Measures (QM/QI) and Case Mix Index (CMI) processes

  • Provide education, mentoring, and ongoing support to nursing and interdisciplinary staff

  • Supervise assessment coordinators and Admissions Associate

  • Participate in committees, audits, daily huddles, and quality initiatives

Qualifications
  • Current RN license in Pennsylvania

  • Minimum of two (2) years of long-term care nursing experience

  • Working knowledge of MDS, Medicare PPS, and CMI

  • Strong organizational, communication, and computer skills
