What you will do
  • Deliver expert pharmaceutical care and optimise medicine use across multiple specialties.
  • Undertake medicines reconciliation and screen prescriptions to ensure safety and efficacy.
  • Support safe and cost-effective prescribing and facilitate timely discharges.
  • Ensure a seamless transfer of care across sectors.
  • Contribute to clinical governance and the development of guidelines.
  • Supervise and mentor trainees and Band 6 pharmacists.
  • Participate in audits and quality improvement initiatives.
  • Collaborate with rotation leads to enhance pharmacy services.
Requirements
  • HCPC registration is required.
  • Ability to drive to ensure you can easily reach patients across different settings.
