Bank Community District Nurse - SD H&C

The Staff Bank for Epsom and St Helier University Hospitals NHS Trust are seeking enthusiastic Community Nurses to join our pool of Staff Bank workers. This role is with Surrey Downs Health and Care (SDHC) which delivers care closer to people's own communities through our Primary Care Networks and our innovative partnership of local NHS organisations.
Shifts are available in the following Primary Care Networks (PCN):
- Dorking
- Cobham and Oxshott
- Leatherhead
Key responsibilities include:
- Work as part of the community nursing team, providing care to patients in their own home or care home setting.
- Assess and develop holistic care packages for patients.
- Perform venepuncture, insulin administration, catheterisation, complex wound management, IV medication administration, PEG care (advanced, rotate, medication and feeds), end‑of‑life care, and palliative care including syringe driver management and symptom control.
- Implement and evaluate care plans.
- Maintain accurate and up‑to‑date records in line with SDHC procedures.
- Support and mentor team members, facilitate training and professional development.
- Participate in SDHC clinical governance and risk management strategies.
- Work independently with occasional direct supervision and liaise with senior team members.
- Ensure compliance with NMC guidelines and maintain professional clinical integrity.
- Adopt safeguards to protect vulnerable individuals and contribute to SDHC Clinical Governance and Risk Management.
- Perform other duties consistent with the post as required.
Professional registration with the Nursing and Midwifery Council (NMC) is required.
