Surrey Downs Health and Care is recruiting a Care Co-Ordinator to work with primary care teams to coordinate and navigate care for patients in the community. You will take referrals, identify those who could benefit from support, and have empathetic conversations with patients, families and carers.
You’ll develop knowledge of local health and care pathways, work with PCN, MDTs and social prescribing, and help patients access the right services.
