As part of our new, competitive LPN wage scale, we offer full-time and part-time opportunities with flexible 8-hour and 12-hour shifts. Key Responsibilities:

• Provide direct patient care including medications, vital signs, treatments, and daily living support

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor and report resident conditions to nursing and medical staff

• Conduct assessments and adjust care plans as needed

• Collaborate with healthcare professionals to ensure seamless care transitions

• Maintain accurate and detailed documentation of care and treatments

• Participate in quality improvement initiatives to enhance resident outcomes

• Provide emotional support, education, and reassurance to residents and families
