Job Summary: The Licensed Clinical Social Worker (LCSW), reporting to the Social Services Program Manager, is an integral member of the Ambulatory Care Management department. Co‑located within primary care offices, the LCSW provides direct care management services to patients, including comprehensive face‑to‑face biopsychosocial assessments and collaboration with primary care physicians to support diagnosis and brief therapy treatment. The LCSW also addresses social determinants of health and assists with advance care planning.

This role primarily serves adult and geriatric patients presenting with behavioral health needs in a primary care setting. Patients referred by their primary care providers are evaluated and treated by the LCSW, who works closely with Care Management team members and Primary Care Providers as part of the interdisciplinary team to ensure optimal outcomes. In every interaction, the LCSW demonstrates respect, empathy, and cultural sensitivity toward patients, families, visitors, and staff.

• Provides clinical social work, focusing on assessment, collaboration with patients and primary care visit to determine diagnosis and treatment of emotional and mental illness related to total health.
• Develops comprehensive psychosocial assessments examining current problems, bio‑psychosocial and environmental factors to establish an appropriate care plan.
• Initiates contact with patient/family in the Physician's office or by phone within one business day from referral date.
• Communicates information on chart in accordance with HIPAA guidelines.
• Creates a positive therapeutic relationship with client leading to correction of dysfunction.
• Provides a wide range of treatment modalities based on assessment, including short‑term CBT, solution‑focused, motivational interviewing, problem solving, crisis intervention, mindfulness, trauma‑informed, behavior activation, acceptance and commitment, person‑centered or other evidence‑based options to stabilize and regulate current dysfunction.
• Identifies social determinants that impair or impact patients' ability to pursue treatment, and works closely with Social Work Department to refer patients for case management needs.
• Develops, implements, and modifies treatment plans.
• Evaluates progress on patients' care plans.
• Terminates treatment process or refers to higher level of care when necessary.
• Initiates patient/family education, counseling and other social work interventions to enhance problem solving and coping.
• Serves as a liaison between patient/family and all members of the healthcare team.
• Acts as consultant to interdisciplinary team regarding psychosocial components, procedures or services to promote greater understanding of the patient.
• Communicates effectively with coworkers and healthcare team to ensure continuity of care and service.
• Integrates knowledge, skill and experience to continuously improve self and quality of patient care.
