Position OverviewThe Licensed Substance Use Disorder Therapist provides clinical care for clients in residential and outpatient settings, including conducting assessments, developing treatment plans, and delivering individual and group therapy. The role also involves collaborating with a multidisciplinary team to ensure effective treatment and recovery.
Key Responsibilities- Clinical Caseload Management:
Manage a caseload of clients, providing assessments, treatment plans, individual therapy, group therapy, and family sessions. - Treatment Planning & Documentation:
Develop and update individualized treatment plans, ensuring documentation meets regulatory and clinical standards. - Multidisciplinary
Collaboration:
Participate in weekly team meetings, share client progress, and coordinate care with support staff and external referral sources. - Supervision & Professional Growth:
Engage in weekly individual supervision with a Licensed Clinical Supervisor to support professional development. - Discharge Planning & Referrals:
Create discharge plans, including safe housing arrangements and referrals to lower levels of care, when appropriate. - Crisis Intervention & Support:
Provide crisis intervention and coordinate with other treatment providers or case managers as necessary. - Community Representation:
Represent Transitions professionally in all interactions with clients, families, and referral sources.
- Master’s degree in Behavioral Health or a related field.
- Active Kentucky license (CSW, LCSW, LPCA, LPCC, LPA) and/or Certified Alcohol and Drug Counselor (CADC).
