GGz Centraal in Ermelo zoekt een betrokken begeleider ouderenpsychiatrie voor de Westerhof op het Veldwijk terrein. Je biedt zorg aan ouderen boven de 60 jaar met somatische en psychiatrische problematiek, gericht op veiligheid, structuur en nabijheid.
Je werkt samen met een multidisciplinair team, stelt en evalueert zorgplannen en ondersteunt ADL-zorg. Daarnaast krijg je ruimte voor professionele ontwikkeling via trainingen en interne scholing.
