MARQUETTE GENERAL HOME HEALTH & HOSPICE - ESCANABA

Responsible for the total direct social services of those patients assigned.

• Maintains established hospital and departmental policies and procedures, objectives, performance improvement program, safety, environment of care, management of information, and infection control standards.
• Utilizes excellent customer service skills at all times.
• Complies with federal and state law and accrediting and licensing agencies at all times, to include but not limited to, JCAHO and federal compliance regulations.
• Provides consultation to others directly concerned with the patient's care regarding techniques to promote optimal use of patient's remaining capabilities and/or rehab gains.
• Provides total individualized social work during visits to all assigned patients of all ages.
• Communicates and promotes working relationships with other team members providing care to the patient.
• Initiates appropriate preventative and rehabilitative procedures.
• Consults with Home Care Coordinator regarding unusual circumstances encountered.
• Maintains updated, accurate records reflecting patients condition and treatments.
• Familiarizes self with current social work trends and shares information with co-workers.
• Initiates and establishes plan of treatment and goals on all patients assigned.
• Evaluates patient's need for social therapy work on a regular basis.
• Informs physician and other health care team members of changes in the patient's condition in relation to social work.
• Enhances professional growth and development through participation in educational programs, current literature, inservice meetings, and professional conferences.
• Participates and attends meetings and in services as required and/or assigned.
• Performs other related duties as assigned or requested.

Minimum Education: Bachelor’s or Masters Degree in Social Work or equivalent

Minimum Experience: 1 year

Preferred Experience: 2-3 years

Required Licensure: State of Michigan Bachelor’s Degree Social Work License –OR
- Registration as a Social Worker and/or licensed in the state providing care as required by state law (Home Care Only)

· Valid driver's license (Home Care Only)

Required Certification: Basic Life Support-Health Care Provider (BLS-HCP)
