Advanced Clinical Practitioner (prescriber)
We are seeking a motivated Advanced Clinical Practitioner to join our clinical team of six GPs, three prescribing ANPs, a prescribing Clinical Pharmacist, three Practice Nurses, two HCAs and a team of GP Assistants.
The successful candidate will use advanced clinical assessment, diagnostic and decision‑making skills to assess, diagnose, treat and manage patients presenting with undifferentiated and chronic conditions in a primary care setting.

Job responsibilitiesKey Responsibilities
You will:
- Conduct comprehensive clinical assessments and manage acute and chronic conditions across the lifespan.
- Diagnose, treat and manage patients presenting with undifferentiated health problems.
- Prescribe and review medications safely and appropriately within your prescribing qualification.
- Order, interpret and act on diagnostic tests (e.g., pathology, ECGs, spirometry).
- Provide same‑day and planned clinics for chronic disease management, minor illnesses and health promotion.
- Support chronic disease registers and practice targets where applicable.
- Deliver opportunistic health promotion and preventative care advice.
- Maintain accurate clinical documentation and adhere to practice governance standards.
- NMC registration (or equivalent).
- Demonstrated autonomous clinical assessment, diagnostic and decision‑making skills.
- Experience managing patients with a range of acute and chronic conditions.
- Excellent communication, organisational and team working skills.
- Ability to work effectively within multidisciplinary clinical settings.
- Experience of working in primary care - desirable
- Experience of working in a GP practice - desirable
