Location: Molesey

The Community Team visits patients in their own homes as well as care homes. The role offers opportunities for training and development of clinical and leadership skills, supported by experienced Senior District Nurses.

Working over a 7‑day week, from 08:00 to 20:00, the post features a variable shift pattern, including weekend and bank holiday working.

Responsibilities
  • Participate in assessment and care planning of patient care in conjunction with the District Nurse.
  • Implement and evaluate care and treatment agreed with the patient and defined in the care plan.
  • Escalate any deviation from the care plan or change in condition to the District Nurse or GP as appropriate.
  • Communicate effectively with patients, carers and multidisciplinary team members.
  • Prioritise daily work in line with clinical and patient needs.
  • Maintain accurate and legible patient records in line with professional and organisational requirements, including contemporary paper and electronic systems.
  • Ensure evidence‑based care and best practice are followed.
  • Actively participate in team meetings and daily handover.
  • Initiate health promotion activities with individuals, patients and carers.
  • Takes responsibility for a defined caseload in the absence of the District Nurse, under the direction of the Practice Teacher or Team Leader.
  • Actively contribute to the development and implementation of community nursing service initiatives.
  • Participate in clinical audit and implement changes to practice based on findings and recommendations.
  • Engage in risk assessment and promote the safety and well‑being of patients, staff and others.
  • Report all complaints, incidents, accidents and areas of clinical risk to the District Nurse in accordance with organisational policies.
  • Maintain professional development through completion of mandatory training, supervision, peer review and annual appraisal.
  • Access and use regular 1‑to‑1 meetings and mentorship training to support learning for students and staff.
  • Accountable for own professional actions in line with NMC code and organisational policy.
  • Monitor and order stock levels and equipment, ensuring infection control and safety standards are maintained.
Qualifications
  • 1st Level Nurse Registration (live registered with the NMC).
  • Adult Nursing qualification.
  • Evidence of continuing professional development.
  • Willingness to undertake further education in management of long‑term conditions.
Skills and Competencies
  • Current driving licence and access to a car during the working day.
  • Ability to travel across bases and home visits.
  • Flexibility and adaptability in a community setting.
  • Knowledge of recent developments within health and social care.
  • Up‑to‑date with evidence‑based practice and research.
  • Ability to operate community equipment and to identify and pass on urgent client information.
  • Planning and organisational skills to manage a weekly caseload and prioritise visits.
  • Proficiency in manual and electronic data entry according to organisational policies.
  • Physical ability to travel efficiently within the locality and to perform basic clinical skills such as venepuncture, IV therapy and suturing.
  • Teamwork and ability to work independently.
  • Excellent listening, communication, presentation and teaching skills.
