We are looking for an experienced and dedicated and skilled Physical Therapist to join our team.

• Conduct physical therapy assessments and evaluations in the home setting.
• Develop personalized treatment plans based on patient needs and goals.
• Provide hands-on therapy, including exercises, manual therapy, and modalities.
• Educate patients and caregivers on exercises, safety, and injury prevention techniques.
• Document patient progress, treatment plans, and outcomes in accordance with company policies.
• Collaborate with other healthcare professionals, such as nurses, occupational therapists, and physicians, to provide comprehensive care.
• Adhere to state and federal regulations for home health therapy services.
• Perform other duties as assigned.

• Valid Physical Therapist (PT) license in New Mexico.
• Previous experience in home health care or outpatient settings preferred but not required.
• Ability to work independently with strong time-management skills.
• Excellent communication and interpersonal skills.
• Compassionate, patient-centered approach to care.
• Valid driver’s license and reliable transportation required.
• CPR and First Aid certification preferred.
