Summary

The CNS/NP serves as a clinical resource and independent practitioner who diagnoses and manages acute and chronic diseases on the inpatient unit. Proactively manages the care of patients across the continuum of care and oversees the case management of complex patients.

Responsibilities
  • Completes a comprehensive health assessment including health history and physical exam.
  • Determines the need for and interprets relevant screening and diagnostic tests.
  • Demonstrates sound clinical judgment and diagnostic reasoning to formulate a diagnosis.
  • Modifies assessment techniques according to patient condition, culture, age, and stage of development.
  • Communicates health findings and diagnosis to the patient, discusses prognosis, and establishes a plan of care.
  • Manages acute and chronic health conditions with pharmacological and non‑pharmacological interventions, assisting patients in following the treatment plan.
  • Prepares written prescription orders, medications, and controlled substances per State of NM and Federal DEA regulations.
  • Initiates timely consultation, referral, or collaboration with the hospitalist and the health care team.
  • Implements health promotion strategies tailored to patient age and culture to reduce risk factors and prevent disease.
  • Maintains good interpersonal communication with patients, families, and team members.
  • Maintains patient medical records as required by hospital and regulatory agencies.
  • Performs other duties as assigned.
Skills & Experience

Minimum two to three years of experience in an acute care setting; prior experience in a hospitalist setting desirable.

Knowledge of professional medical practice, organizational policies, regulations, and patient care equipment. Proficiency in maintaining medical records and establishing effective working relationships. Ability to remain calm and respond effectively in emergency situations.
