A community support organization in Estevan is seeking a Transition Consultant to assist youth with disabilities in transitioning from high school to post-secondary education or employment. Responsibilities include providing support, developing individualized plans, and liaising with educational institutions. Candidates should have relevant post-secondary education and experience with youth. A valid driver's license and the ability to travel within the region are required.
