• Job Description: Provides case management services, which includes assessing, planning, monitoring and evaluating for determining the care needs of clients, collaborative coordination of services and case management process. Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement

• Bachelor degree in Social Work, Psychology or other health related discipline

• Valid Class 5 driver's license

• Bachelor Degree in a health related field
