Position: LPN for Long Term Care Facility

Become a valued member of the health care team as a Licensed Practical Nurse. Promote healing by providing care, education, and emotional support to Long Term Care residents.

Your primary responsibilities will include routine assessments and developing individualized care plans for residents. You will perform medical tasks, such as checking vital signs and giving medications, while also delivering specialized care for chronic conditions. Collaboration with health professionals is key to ensuring optimal outcomes.

Key Responsibilities:

• Assess and implement resident care plans

• Record vital signs and administer medications

• Offer emotional and educational support to families

• Administer specialized treatments for long-term conditions

• Collaborate within a multi-disciplinary care team

Requirements:

• Diploma in Practical Nursing required

• LPN certification mandatory

• Eligible/registered with CLPNS

• Demonstrated organizational and interpersonal skills

• Basic computer proficiency needed
