California Licensed RN Clinical Coordinator

To provide critical clinical coordination and operational support, the full-time California Licensed RN Clinical Coordinator will manage patient caseloads, ensure compliance with documentation, and support field clinicians in a remote capacity.

Key Responsibilities Provide care coordination and clinical oversight for assigned patient caseloads across both agencies Review, audit, and sign clinical documentation, including Starts of Care (SOC) and Discharges Serve as the point of contact for incoming clinical inquiries from field staff and patients, offering guidance and escalating complex issues as necessary

Required Qualifications Active, unrestricted California Registered Nurse (RN) license Minimum of 1-2 years of hands-on home health RN experience Minimum of 1 year of supervisory experience in home healthcare or a similar field Strong proficiency with Kinnser/Well Sky EMR preferred Ability to fulfill the duties of this role remotely with strong digital communication skills
