Schedule:

80 hours every 2 weeks = 1st shift (7:30a-3:30p) Monday through Friday Position Summary

Under general supervision of the Site Leader performs duties related to the overall care of consumers in assigned day support or vocational program. Must be able to successfully complete all of the functions and competencies associated with this position as well as other duties as they may be assigned.

Essential Job Functions
  • Assure individuals have opportunities for enrichment, learning and skill development daily within the program and community offering daily opportunities for individuals to connect with people outside of the program and access the community around them.
  • Assist individuals with preparing, packaging, and labeling produce grown at HELP Harvest Greenhouse
  • Consistently acts with the utmost respect for human dignity and human rights, safeguarding the confidentiality of program participants and advocating with individuals for the advancement of their rights and recognition within the community.
  • Provide direct daily shift coverage in program, aiding with activities of daily living as identified in the ISP (Individual Support Plan), maintaining required ratios, following scheduled assignments away from the program and implementing program routines to assist participants in connecting effectively in their community.
  • Develop community connections that result in supportive relationships as well as enrichment activities.
  • Responsible for providing transportation using agency vehicles (including 15-passenger vans) to and from individuals' residence and community integration.
  • Work with the individual to assess interests and needs and implementing what is important to and for the individual.
  • Work with program management to develop and teach curriculum based on age-appropriate activities, ongoing learning, and healthy lifestyles.
  • Help program participants develop new health, wellness, and life skills (i.e., exercise, reading and math skills, money counting, cooking).
  • Provide consistent and accurate implementation of individual and program protocols, completing required data collection in a timely fashion. Use information to track progress and continually build skills.
  • Assure individuals have access to volunteer opportunities that meet their needs and interests, providing the necessary accommodations within the community.
  • Provide active treatment, emotional and health-related services including administering medications, assisting in breathing treatments, checking vitals to monitor overall health, recording medical histories, and assisting with any required medical equipment. Accurately administering medication and following and maintaining MARS.
  • Assist with providing basic personal care and grooming, including bathing/personal hygiene, basic hair care and getting dressed and other assistance to consumers
  • Transport consumers in Agency and/or personal vehicles
  • Responsible for administrative task associated with providing support.
  • Immediately report ALL incidents or accidents (UIR and MUI situations) to the Site Leader.
  • Accurately, legibly, and cohesively document all services provided to consumers
