Outpatient-only practice in a collaborative, team-based care model Minimal clinic call EPIC EMR system On-site lab and X-ray services Multidisciplinary team includes:
Family Medicine: 3 physicians, 3 NPs, 1 PA Internal Medicine: 1 physician Geriatrics: 3 physicians, 6 NPs Pediatrics: 5 physicians In-clinic support from social worker, pharmacist, chronic disease coordinator, and care manager Stable, mission-driven team with long-standing support staff
