Position Summary: The MDS Coordinator works collaboratively with the interdisciplinary team to assess resident needs, ensure accurate MDS completion, and maintain compliance with state and federal regulations.
Key Responsibilities- Participate in the pre-admission process to gather essential information for MDS completion
- Review resident care plans and assist in setting the appropriate Assessment Reference Date (ARD) for PPS MDS assessments
- Ensure timely and accurate completion of resident assessments in compliance with state and federal guidelines
- Accurately code MDS data using information from staff interviews, resident and family input, observations, and medical record reviews
- Maintain MDS schedules and deadlines
- Perform other duties as assigned
- Current, active RN or LPN license in the state of practice
- Strong interpersonal and communication skills
- Knowledge of the RAI process and case management
- Prior MDS experience preferred
