In-Home NP​/PA | Part-Time Mission-Driven Care

Signify Health, Inc. is seeking a part-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for Medicare and other vulnerable populations in patients' homes in Eugene, Oregon.

You will complete health risk assessments, review medications, address care gaps, educate patients on chronic disease management, document encounters, and collaborate with remote clinical teams to support value-based care.
